Williamsburg Landing, Riverside Partner for In-Home Aging Program

logosRiverside Health System and Williamsburg Landing are partnering to offer an aging in place program to local residents.

Riverside’s ChooseHome program, which allows independent adults to stay in their homes while having access to a safety net of care services, is offered to adults ages 60 and older wishing to plan for long-term care needs.

ChooseHome works with clients to provide individualized support that considers financial needs as well as health concerns.

Rather than physician house calls, the program enlists a “personal services coordinator” to plan and coordinate care based on members’ goals.

ChooseHome was the first of its kind in Virginia in 2012, after legislation allowed for the creation of Community-Based Continuing Care in the Commonwealth.

“Our vision is to help people age in a place of their choosing,” said Bob Bryant, senior vice president for lifelong health and aging related services at Riverside, in a news release. “With ChooseHome, we’re able to offer residents in our community the ability to stay in their homes and still have access to the high quality services and healthcare they need, while also protecting their finances.”

In joining with Williamsburg Landing, a center designed to accommodate its residents’ needs through multiple stages of life, ChooseHome can utilize existing technology and care services there, including access to assisted living or nursing care if needed in the future.

Williamsburg Landing President and CEO Steve Montgomery said in the release the expanded partnership between his retirement community and Riverside – which has provided physician services there for more than three years – aims to give more residents access to ChooseHome.

“Our Board of Directors has been assessing the growing numbers of seniors in Williamsburg and realized the need for additional services to help older adults as they age in place,” he said. “A joint venture with Riverside to sponsor ChooseHome is the best solution to help the seniors in the community.”
